What Constitutes PHI?

According to the HIPAA Privacy Rule, information is considered health-related if it pertains to:

An individual’s past, present or future physical or mental health or condition, or:

Any provision of health care to the individual, or:

The past, present, or future payment for the provision of health care to the individual

That health information becomes PHI when it’s combined with one of 18 attributes that could be used to identify the individual:

  • Names
  • Geographic identifiers
  • Dates (except year) directly related to an individual
  • Telephone numbers
  • Fax numbers
  • Email addresses
  • Social Security numbers
  • Medical record numbers
  • Health insurance beneficiary numbers
  • Account numbers
  • Certificate/license numbers
  • Vehicle identifiers and serial numbers, including license plate numbers
  • Device identifiers and serial numbers
  • Web URLs
  • Internet Protocol (IP) addresses
  • Biometric identifiers, including finger and voiceprints
  • Full face photographic images and any comparable images
  • Any other unique identifying number, characteristic, or code
